Send the form , supplemental sheets to support the award claim, supporting letters and documents by e-mail . Please make a folder named as follows : Name of the Nominee followed by Awards 2011.Then Zip the folder and e-mail to dentistry2011@idrr.org with subject line AWARDS2011.

1. Nominee and Nominators should be registered dental surgeons with Indian Citizenship for DENTIST EXCELLENCE AWARD INDIA the nominee must have contributed to the Dental Academics

2. DENTIST OF THE YEAR 2011 (Nominee and Nominators should be registered dental surgeons practitioners/academicians from any part of the world and Nominee should have contributed to the clinical dentistry)

3. OUTSTANDING YOUNG DENTIST AWARD(those dentists below the age of 40 from any part of the world who proved their mettle in non dental fields. May be a musician ,actor or any such category ) .

4. Nominees for DENTIST FRIENDLY AWARD must be non dentists /dentists who have contributed immensely to the growth of dental industry in India.

5. BUDDING DENTIST AWARD nominations should be submitted in favor of dental students /interns who are students from any Dental College in India.

6. BEST CLINICAL PAPER AWARD : Must be a general practitioner. This must be related to the practice of clinical dentistry. Clinicians may submit an abstract of case reports, topic updates or creative solutions to clinical problems that they have encountered in practice. The selected abstracts have to be presented in full as a power point presentation during DENTISTRY 2011.

QUALIFICATIONS

Each award has a different set of qualifications.
1. For Awards 1-5 :
Identification and evaluation of the accomplishments on which the nomination is based (Exactly 1000 words ).
2. If the nominee has previously received any award from Indian Dentist Research and Review, and explicit statement of new accomplishments or work over and above those cited for the earlier award(s) must be included (maximum 500 words ).
3. Selected bibliography (including books and major papers published), where ever applicable.
